The Healthcare Access program offers short-term crisis intervention, providing clinical case management and mental health services to stabilize individuals and refer them to long-term care. Healthcare Access focuses on historically underserved populations, offering group therapy and support in trusted, culturally sensitive environments. It also provides reentry support for individuals who may have disengaged from care.
Problem Statement: Historically under-resourced communities often face significant barriers to accessing timely and culturally competent mental health care. Many individuals experience gaps in services, lack of trust in traditional healthcare systems, and limited access to long-term support. Without effective crisis intervention and linkage to appropriate resources, these communities remain vulnerable to untreated mental health issues and broader systemic challenges, leading to ongoing cycles of instability and poor health outcomes.
Solution Statement: Healthcare Access program, funded by CalAIM programming, medical billing, and grants, bridges this gap by offering short-term crisis intervention, clinical case management, and mental health support tailored to the needs of historically under-resourced communities. The program provides culturally competent, trauma-informed care through Community Health Workers, Lead Care Managers, and Clinical Consultants, who stabilize clients and link them to long-term services. By delivering transitionary mental health services, safe group therapy modalities, and reentry support, Healthcare Access empowers individuals to regain stability and facilitates their access to ongoing care through trusted community partners. This holistic approach improves overall health outcomes and strengthens trust in healthcare systems.
Program Goal: Deliver short-term crisis intervention and clinical case management for historically under-resourced communities, focusing on stabilizing individuals in need and linking them to long-term mental health services. The program addresses gaps in traditional healthcare systems by offering culturally competent, trauma-informed care and reentry support. It aims to empower individuals to regain stability, improve access to mental health services, and foster trust in healthcare systems, ultimately improving overall health outcomes in marginalized communities.

- Program referrals can come from healthcare institutions, community-based organizations, schools, or other agencies identifying individuals in need of short-term crisis intervention and support.

- Problem: Alternative healing practices have shown significant potential in promoting well-being, restoring balance, and sparking personal transformation. However, these practices are often overlooked or marginalized by traditional healthcare systems.
